Application & Instructions
To apply for Protégé Awards, Merit Awards, or Need Awards, do the following:
1) Read all information in the Awards section of this site, especially Awards -- Overview of the Application Process. Also read these Important Site Disclosures and Disclaimers.
2) Download and complete the application below.

(This address is not included with the application form, so be sure to make a copy of the address for yourself!)
Remember: ALL application materials for 2004 selections must be:
A) Post-marked no earlier than January 1, 2004,
B) Post-marked no later than February 1, 2004,
AND
C) Received by Quaqua no later than February 15, 2004.
Applications sent at the incorrect
time or in an incorrect manner will be discarded.
Unsuccessful applicants may reapply for the Protege Award, so long as they are otherwise eligible, even if they have previously received some other award from Quaqua. A recipient of a Quaqua award other than the Protege Award is ineligible for any subsequent Quaqua Award, except that they remain eligible for the Protege Award, the Elijah Award, and Parent of the Year Award.
Absent an announcement to the contrary, the same procedures shall apply for 2004 and subsequent years. The Society reserves the right to enforce all guidelines according to its own sole discretion. Procedures for nominated candidates are according to the discretion of the Society and are not necessarily confined to the same procedures and deadlines used for applicant candidates.
